Ask your physician to assist you in determining your eligibility for Home Health Care. To be eligible, you must be essentially homebound; have a plan of care established by your physician; and be medically determined to be in need of the services outlined in the plan of care.
Southwestern Illinois Visiting Nurse Association provides private, supplemental care for clients who wish to remain independent in their own home. This care is provided through our Private Duty Home Care Division. We understand a person's need and desire to remain in their personal surroundings and we strongly believe a person's physical, emotional, social, and spiritual needs are best met in the comfort of their own home. Different services are paid for through different channels. Medicare, Medicaid, and Third Party Private Insurance pay for the skilled services or a registered nurse, physical therapist, occupational therapist, speech therapist, medical social worker, and home health care aides. SIVNA accepts private payments for services such as blood draws, when not covered by any payor source, immunizations, and private duty home health care services.
In most cases, SIVNA will take assignment on insurance claims, which means we accept what the insurance company designates as the payment amount for the service they cover, saving the patient the 20% out-of-pocket expense.
SIVNA's medical personnel work 7 days per week to make sure you're taken care of if the need arises. Your physician or his/her office staff simply calls Southwestern Illinois Visiting Nurse Association, informs us of your needs, and a SIVNA registered nurse will contact you for the most convenient time to see you.
A registered nurse is available for medically related consultation 24 hours a day.
